I was getting that message every time i made a call. after I treid disabling forwarding on the phone. (everything was already disabled) it turned out that it was the network providers setting. I used Aldi which uses the Telstra network and I had set the ring time from standard 15 seconds to 30 seconds and that was the que to get that conditional forwarding message. I reset that back to 15 seconds by ##004# send, and MMI reset came up with call forwarding disabled. message is gone. You might have to go to your network providers website for yours. Good luck.
